Patient Registration

Your time and convenience are important to us. To help expedite the registration process, pre-registration for scheduled appointments is currently available over the telephone by calling (989) 907-8110.

Because people move, get married and change employers, your demographic information will be reviewed each time you register for services at St. Mary’s of Michigan. Be sure to bring your current insurance cards, social security number and emergency contact information with you.

Consent forms

Upon arrival, you will be asked to sign Consent to Treatment form. This allows the hospital to provide your care, bill your insurance, ensure you have been notified of your right to privacy, assignment of insurance benefits, financial agreement and provides an authorization of release of information. Depending on your type of insurance, treatment or procedure you may also be asked to sign additional forms.

If you do not fully understand what you're being asked to sign, do not hesitate to ask a staff member for clarification.
Permission for treatment of minors must be signed by parents or legal guardians.

Identification bracelet

At the time of registration, an identification bracelet will be placed on your wrist. This is for your safety and helps our staff clearly identify you. Please wear it throughout your stay.

PIN (Patient Identification Number)

You will also be given a personal identification number (PIN). Your family members will be asked to provide this number when calling for information regarding your stay.
